Welcome to Patient-Centered Health, a single-provider primary care office located in Clearfield, Pennsylvania. Dr. Diane Hoover is a Nurse Practitioner with 19 years of experience in healthcare. Our model of care is centered around respect for patient preference and coordination of care, education, physical comfort, emotional support, involvement of family or friends, continuity, and access to care.

Our patients tell us that they’re frustrated with long wait times to get in to see a clinician, waiting too long once they’re in, and a feeling of inadequacy in the healthcare provided. That’s why we decided to open our own practice—to offer the local community and surrounding area more options. Many of our patients are waiting 2-3 months to get in to see a clinician, and we want to give each individual the time they need to tell their story and become involved in decision-making regarding their health.

We provide a range of essential services, such as annual wellness visits, acute visits, follow-ups, chronic care management, flu vaccinations, pneumonia vaccinations, and shingles vaccinations, as well as CLIA-waived testing, such as urine dipsticks, strep testing, and blood glucose monitoring. Our goal is to educate, support, and manage chronic disease in an efficient and patient-centric way.

We want to make it as easy as possible for you to get the care you need, and we’re committed to giving every patient more quality time to manage their overall health with a focus on education and respect. Insurance is accepted including: Medicare, Medicaid, Humana, Cigna, Tricare, United Healthcare, Highmark, UPMC, Health Partners, and Geisinger. We also, of course, accept self-pay.
